Hours Office Mon-Fri 9 am-5 pm * service hours are flexible depending on the needs of individual clients
Eligibility Aging adults and adults with special needs or those who are isolated * Durham Region residents being discharged from hospital * includes Durham Region residents discharged from hospitals outside the Region
Application Referrals generally come from hospital staff but clients can self-refer by phoning

Description
Description (Service) Home At Last 
Help aging adults and adults with special needs to settle in safely at home following a stay at hospital * certified and specially trained Personal Support Workers drive or accompany the patient home * provide personal care, prepare a small meal, pick up groceries/prescriptions and perform light housekeeping as required the day of discharge * follow up with phone calls and/or visits to check on client's well-being and make referrals to other community support services 
 
Access to Primary Care  
Provides Personal Support Worker (PSW) accompaniment to high-risk older adults who require support to attend primary care appointments or assessments
