Description
Description (Service) Community-based, multi-service agency helps people live at home with a network of support * clients with needs related to aging, physical and/or mental health in Durham Region * Home Support, Respite, Adult Day Programs, Assisted Living Services, COPE Mental Health Support, Home At Last (HAL) through local community offices serving the communities of Ajax-Pickering, Brock, Clarington, Oshawa, Scugog, Uxbridge and Whitby 
 
Home Support  
Meals on Wheels 
Nutritious hot or frozen meals delivered by volunteers to clients in their homes  
 
Transportation 
Drives available primarily for medical appointments, and when time allows for shopping and errands 
 
Home Help 
Assistance with light housekeeping  
 
Home Maintenance 
Yard work, snow shoveling, occasional minor repairs 
 
Luncheon Out 
An opportunity for clients to enjoy a nutritious meal and entertainment in a social setting 
 
Friendly Visiting 
Volunteers visit on a regular basis to share in activities and provide companionship to clients 
 
Telephone Reassurance 
Daily Monday to Friday telephone calls to clients for a friendly check-in and brief chat. Checks on their well-being and reassures them that help is available if and when needed 
 
Foot Care Clinics 
Services are provided by registered foot care nurses for those who cannot manage their own foot care 
 
COPE Mental Health 
 
Group Support 
Trained staff and volunteers facilitate support, coping skills, educational and social/recreational groups 
 
In-Home Respite  
Personal Support Workers (PSWs) are trained to maintain a safe environment and provide personal care, assistance with the activities of daily living, social interactions, meal preparation, client-centered light homemaking 
 
Adult Day Program  
Safe and comfortable setting for clients to enjoy a wide array of social and recreational programs, and at the same time provides a break for caregivers 
 
Assisted Living Services (ALS) 
High-risk seniors who can reside at home, but may require personal support, homemaking, security checks and reassurance services on a scheduled and unscheduled 24/7 basis * available in limited areas in Ajax, Clarington, Oshawa and Whitby 
 
Home At Last 
Assists clients at no charge in settling in their homes safely and comfortably after a stay in hospital
